What is the Home Hospital?
The Silver Chain Home Hospital (external site) is a metropolitan based service made up of four interlinked components.
- Priority Response Assessment (PRA) provides a 24 hour, 7 day a week nursing service that is supported by a network of GPs.
- Hospital at the Home (HATH) is a hospital substitution program that is a 24 hour, 7 day a week service. The service generally follows an admission to a public hospital.
- Post Acute Care (PAC) Services are nursing services provided in the immediate post-discharge period from a public hospital, PRA or HATH episode of acute care.
- Community Nursing (CN) provides an alternative to hospitalising patients who need nursing interventions at home and who are not eligible for Home and Community Care (HACC) nursing.
Home Hospital services are available to the community, including residential care. Referral is through a general practitioner, registered nurse, public hospital and associated hospital programs.
Cost
There is no charge for users of this service.
Who is the Home Hospital for?
Home Hospital services are for any person over 18 years of age living in the community or in residential aged care who has acute or post acute care needs that can be treated effectively and safely in the persons place of residence. Referrals are normally through a public hospital, GP or community nurse. The referral criteria and process can be viewed on the Silver Chain Home Hospital web site.
