Interprofessional Post Fall Assessments for Occupational Therapists and Physiotherapists

This model is where a post fall assessment is completed by one health professional but covers the disciplines of Physiotherapy and Occupational Therapy. It aims to deliver an efficient and coordinated service that is informed by evidenced based best practice and provides assessment and intervention for patients to ensure safe and timely discharge from the hospital and prevention of clinical deterioration prior to admission.

The model has been adapted from that which the Fiona Stanley Hospital, Perth, Western Australia uses.
