Health Networks Supported Programs

Health Networks provides funding for a number of ongoing programs and pilot programs which support the continuous improvement of health care services across WA. We support non-government organisations to deliver pilot programs aimed at improving the health and wellbeing of West Australians.

Health Networks supports evidence based pilot programs to provide the opportunity to assess innovative service delivery models before they are taken up by Health Service Providers for roll out across WA. All of the ongoing Health Networks programs began as highly successful pilot programs.

Ongoing Programs

Carer Participation and Representation

Carers WA provide and deliver support for carers to increase their participation in committees and other decision making bodies with the WA health system. They ensure that carers of health consumers are adequately trained and supported to provide carer representation and participation at all levels of planning, service delivery, and evaluation within health services.

Healthy Lifestyle Program

The Healthy Lifestyle Program targets people at risk of, or newly diagnosed with, a chronic disease. It supports people to improve the management of their health and wellbeing, and the risk factors that lead to poor health. The program is available in the North Metropolitan and South Metropolitan Areas.

Homeless Health Care

The Homeless Health Care program assists people experiencing or at risk of homelessness in the metropolitan area to receive appropriate and timely primary health services. In addition, people are referred to other health and specialist services as appropriate so that they experience the best possible health outcomes.

painHEALTH 

painHEALTH (external site) is an online consumer oriented resource for the self-management of musculoskeletal pain. The aim of the website is to help health consumers with their musculoskeletal pain, access reliable and usable evidence-informed information, and learn skills to assist in the co-management of their musculoskeletal pain.

Pilot Programs

Diabetes Complex Care Clinic Pilot

The Diabetes Complex Care Clinic (DCCC) is a pilot project to improve community based treatment for people with complex diabetes. The program builds a network between a patient’s GP and a multidisciplinary team in the DCCC which includes advanced trained GPs, diabetes educators, dietitians, and exercise physiologists.

The DCCC team provide advanced interventional care and advice to patients and supports their regular GP to follow through on their management plan. The program delivers high quality, complex care while significantly reducing the patient’s wait time for a consultation and frees space in hospital outpatient clinics for patients who require specialist care.

Let’s Prevent Pilot Program

The Let’s Prevent pilot program supports people at high risk of developing chronic conditions such as cardiovascular disease, stroke, and/or diabetes in regional WA. It is an early intervention initiative designed to educate, inform, and empower people to achieve long term adoption of positive health behaviours.
