HACC funded service providers are required to submit the following service reports annually by the July deadline as stipulated in the current Service Agreement. Submit to your Project Officer and identify your organisation in the subject heading of the email.
Carers Recognition Report and Disability Access and Inclusion Plan are due by 15 July.
Subcontracting Report is due by 31 July.
